Financial Obligation

I, the parent/guardian of this applicant, as well as the player applicant understand that in accordance with PAMHA rules and agreements, all dues and/or fees must be paid by the due date.  Non-payment of any dues and/or fees will result in suspension from team participation and terminations of all membership privileges.  Signing (electronic or in writing) the registration form holds the player/parent/guardian responsible for all of the aforementioned fees.
